Q: How do I go about turning in someone for filing their taxes and claiming a child who does not belong to them, who they do not support?

Answer: Tax fraud cases along these lines can be reported to the Internal Revenue Service using a Form 3949-A document, which you can download at www.irs.gov/individuals/how-do-you-report-suspected-tax-fraud-activity. It should be mailed to Internal Revenue Service, Fresno, Calif. 93888.

You can also send a letter to that same address instead of using that form. If you choose that option, the IRS asks that you include as much information as possible, including:

  • The name and address of the person or business you are reporting.
  • The individual’s Social Security number or the business’ employer identification number, if you have that information.
  • A brief description of the alleged violations, including how you became aware of them.
  • The years involved.
  • The estimated dollar amount of any unreported income.
  • Your name, address and telephone number. “Although you are not required to identify yourself, it is helpful to do so,” according to the IRS. “Your identity will be kept confidential.”

Due to tax return confidentiality rules, people submitting a report will not receive a status or progress update on the return.

Tax fraud cases involving potentially stolen identities are handled differently; you should go to the web site listed above for more information in those cases.

On the state level, you can report suspected tax fraud at www.ncdor.gov/contact-us/report-tax-fraud or by phone at 800-232-4939.

Q: In Surry County, N.C. 89 between Flippen Road and Lowgap was repaved last fall. Are they going to install center line reflective markers in the pavement?

Answer: “The contractor is behind but will be installing the markers as part of the contract,” said Charles Reinhardt, division maintenance engineer with the state Department of Transportation.

Q: On Sunday evening, we were ready to watch the Super Bowl on WFMY over-the-air on channel 2.1, but the signal was gone. Was there a local blackout? I checked the channel on Monday morning and it was working like usual.

Answer: You may have had a problem with your antenna or television, but if there had been a widespread problem, SAM or WFMY would have heard from more people about it.

“We had a flawless evening and are thankful for it,” said Larry Audas, the president and general manager of WFMY. He said the station had been planning for months and had multiple redundancies and backup plans in place in case a problem arose. “We treat these events with extreme care,” he said. “We had zero complaints.”

Locally, the Super Bowl broadcast got an impressive 43 rating and 62 share, Audas said, meaning that 43 percent of households in the market — and 62 percent of the households that were actually watching TV at that time — were tuned in to WFMY.

Overall, the Super Bowl averaged 100.7 million viewers across multiple channels and streaming outlets, according to CNN Business, which said that overall viewership was the lowest for a Super Bowl since 2009.
