Seniors just out of hospital get help with nutrition

When Forsyth County seniors are released from the hospital, they are assessed to determine whether they have help at home so that they can recover -- and decrease their chances for re-admission.

If hospital workers determine that certain seniors need help with housework, need someone to accompany them to medical appointments or need assistance to take their medications properly, the federal Centers for Medicare and Medicaid Services has funds for qualifying patients.
